Children's Health Initiative: Bexar County Report

CHILDREN AT RISK (C@R), Family Service, and the Baker Institute for Public Policy at Rice University partnered to launch the CHI in response to Community Health Needs Assessments that signaled poor mental health, nutrition, and hightened rates of chronic disease for children in Bexar County.

January 2026 – Children’s health does not exist in isolation. It is shaped by access to opportunity, economic stability, education, and the strength of community systems. In examining the demographic makeup of Bexar County, it is clear that this region has a significantly higher Hispanic population, lower median household income, lower rates of college attainment, and higher overall and child poverty rates than the state overall.

With nearly 2.1 million residents, Bexar County represents a snapshot of Texas’s evolving population. It is the 17th largest county in the nation, anchored by the urban core of San Antonio. With more than one million additional residents anticipated by 2040, intentional planning grounded in health equity is essential to address the scale of this expected growth.

Community Health Needs Assessments have consistently identified three urgent and interconnected challenges in the region: children’s mental health, nutrition and related chronic disease, and kindergarten readiness. These issues disproportionately affect Latino children and families and other historically under-resourced communities. In response, the Children’s Health Initiative Bexar County was launched as a strategic partnership between CHILDREN AT RISK, Family Service, and the Baker Institute for Public Policy at Rice University.

Concentrating on the age when disparities most often take root, yet are most responsive to intervention, this research focused on early childhood. The analysis leveraged the Pediatric Vital Signs Framework, census-level data, bilingual field interviews, and existing Community Health Needs Assessment findings.

Together, the data and community insight point to clear priorities. Access gaps persist. Economic instability intersects with service delivery. Families are navigating complex systems without consistent coordination. At the same time, Bexar County possesses strong community assets and a history of cross-sector collaboration that can be leveraged for impact.

Through the Children’s Health Initiative Bexar County, publishing partners provide in-depth analysis and cross-sector recommendations to bridge access gaps, alongside an interactive Resource Map that allows stakeholders to examine the indicators shaping children’s well-being.
